Primary Care Manager Continuity

What Are We Tracking And Why? 

We keep track of how often you are seen by the same Primary Care Manager (PCM) or Primary Care Medical Home (PCMH) team at each primary care visit. A healthcare team that is familiar with your medical history can make more informed decisions about your care, especially if you have more complex medical issues. Seeing the same healthcare providers gives you an opportunity to receive more complete, safer and higher quality care.

What Are We Doing To Improve? 

We have standardized our approach to building and sustaining our Primary Care Medical Homes (PCMHs). We seek to meet or exceed the National Committee for Quality Assurance (NCQA) and The Joint Commission (TJC) guidelines for PCMH standards. In the event that we are unable to provide you an appointment with your PCM, we will make every effort to arrange a visit with a member of your PCMH team to maintain continuity of your care.

What Can You Do?  

Be an active member of your healthcare team. When you have healthcare questions, be sure to contact your PCMH , or contact the TRICARE Nurse Advice Line at 1-800-TRICARE (874-2273), option 1, if calling after hours. Enroll in Secure Messaging to have direct access to your PCM.

When making an appointment, ask to be seen by your PCM or a member of your PCMH team.

If your PCM deploys or changes duty stations, you can take comfort in having a Primary Care Medical Home. Your PCMH team is an extension of your Primary Care Manager, and your team will work together to provide a seamless transition with a new PCM.
